About NDIS Support Coordination

Meet Sandy, Independent NDIS Support Coordinator

If you are looking for a support coordinator that will go that extra mile for you then you have come to the write place.

With over 20years experience in the disability sector you will have a highly experienced professional navigating you through your NDIS journey

The NDIS can be extremely daunting to people when they have received an NDIS plan.

Questions that I am generally asked is "How do I do this?" "What if I spend my funds on the wrong thing?" "What can I use my funds for?" "What if I don't use my funds?' "How do I connect with Service Providers?"

When you onboard with me I take the difficulty out of the decision making by explaining what your funds are, how the funds are managed, connection with services, referrals, break down of YOUR budget and make sure you receive the best value for money.

It is my job to make sure you receive the very best of a wrap around approach, that means all services working together in providing the best stress free service available in supporting you or your loved one to have a positive experience.

Too often I have participants and their families contact me because they feel lost. They have not had positive experience with service providers or have not known how to engage correctly to receive services.

This is where I take the difficulty out of the process.

Step 1. I will meet with you for an introduction.

During the induction we will discuss your goals and aspirations and how they are in line with your NDIS plan. We will take a look at the things that are needed to give you the most independent life possible. Discuss barriers that you may feel you are facing and what is important to you.

The first step is making sure that you are feeling comfortable and confident with sharing your information with  me.

This is a very imporant step as to assure that all stakeholders, such as yourself and the people that are important in your life are able to voice and have their say.

Step 2.With premission I will view your NDIS Plan and explain to you hat your funding means. There are three general coponents to NDIS funding. These are CORE. CAPACITY and CAPITAL funding.

Core is flexible funding that can be used flexibly. What these means is that  these funds can be used for things such as community access, in-home daily supports and things that you require on a daily basis such as assistance with going out in the community, have support workers help you while at home and other things that may be required on a daily basis to promote YOUR independance.

Capacity these funds are what can be used to build your capacity. They may assist in thigs such as Allied Health Therapies.

OccupationalTherapists/Physiotherapy/Psychology/Counselling/Speech Therapy and other therapies are included in capacity. Often capacity is only flexible within those required services or only for specific things.

Capital funding is used generally for larger items that may be required such as AT (Assistive Technology) These are items that need to be prescribed through appropraite NDIS Professionals and approved through the NDIA. Items such as wheelchairs, home modifications, ramps or items that cannot be purchased though low-assistive AT.

Step 3. I will then discuss what your funding means for you, and identify where your needs are and are not being met. It may be that you are needing addittional support in added areas so that you are able to build your capacity.

If you decide to take me on as your Support Coordinator I will then discuss the service agreements, schedules of support and relevant information that you need to know such as the Support Coordinators roles and responsibilities and what your own roles and responsibilities are.

I will then create a support plan to suite your needs, meaning that you have full input to what services are provided for you.

Step 4. I look at your preferences, needs and wants then I get to work.

I will connect the services that you may need. Service providers such as support workers, Physiotherapists, Occupational Services, Speech Therapists, Psychologists and other allied health professionals. As a Support Coordinator it is my role to have service agreements in line with providers to make sure they are in place for the length of your NDIS Plan. It is the position of the Support Coordinator to write the reports and collect the evidence and documentation that is required if you are needing any changes in your NDIS Plan.

Apart from utilizing your NDIS Plan it is also my job to link you inn with community services. It could be that you require assistance with housing or are unhappy with a service you are receiving? I go to bat for you.

I love NDIS Support Coordination because their leaders are progressive, and they always want to satisfy my needs.
